Johns Hopkins Community Health Partnership

An Evidence-Based Practice

Description

The Johns Hopkins Community Health Partnership (J-CHiP) aimed to improve care coordination in East Baltimore, Maryland. The partnerships target population was Medicare and Medicaid beneficiaries. The program had two principal components: (1) a bundle of interventions deployed in 2 acute care East Baltimore hospitals (Johns Hopkins Hospital [JHH] and Johns Hopkins Bayview Medical Center [JHBMC]), with additional focus on those discharged to local skilled nursing facilities (SNFs) and (2) a care management model embedded in ambulatory primary care sites located in the community. This program was implemented from 2012-2016 through the Health Care Innovation Award (HCIA) from the Center for Medicare & Medicaid Innovation, a component of the Centers for Medicare & Medicaid Services.

Goal / Mission

The John Hopkins Community Health Partnership's (J-CHiP) goal is to improve care coordination with Medicare and Medicaid beneficiaries.

Impact

The John Hopkins Community Health Partnership participants saw lower spending and improved health outcomes in regards to hospital admissions, re-admissions, and emergency department visits.

Results / Accomplishments

Medicaid beneficiaries saw reductions per 1,000 enrollees of hospitalizations (33), ED visits (51), 30-day readmissions (36), and avoidable hospitalizations (7). Medicaid beneficiaries had statistically significant reductions in total cost of care compared to a comparison group (average reduction of $1,643 per beneficiary per quarter, not accounting for the cost of the intervention). There were no significant differences in results for Medicare beneficiaries.
(Note: another study on a smaller sample of J-CHiP participants conducted by Murphy et al., 2018 found no significant differences in outcomes of interest for Medicaid or Medicare beneficiaries.)
