The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

Goal: The goal of the program was to recognize and address the diversity of local health disparities by marshaling local community involvement in the place-based Health Equity Zones.
Impact: The framework established through the Health Equity Zones allows for the continued collaboration between governmental public health entities and stakeholders in the community to address health disparities.

Goal: The purpose of the first component, ROP, is to develop a strong sense of African-American cultural pride and ethnic identity in the participants and instill a sense of responsibility in their community, their peers, and themselves. The second component, the JTP experience, places youths in summer jobs at desirable work sites such as dentist offices, local museums, and recreational centers. The third component, JA, teaches how to develop and implement a small business.

Comparative Cost Analysis of Housing and Case Management Program for Chronically Ill Homeless Adults Compared to Usual Care (Chicago, Illinois)

Goal: To assess the costs of a housing and case management program in a novel sample: homeless adults with chronic medical illnesses.
Impact: Compared to usual care, the intervention group generated an average annual cost savings of (−)$6,307 per person (95 percent CI: −16,616, 4,002; p = .23). Subgroup analyses of chronically homeless and those with HIV showed higher per person, annual cost savings of (−)$9,809 and (−)$6,622.

Goal: To assess the effectiveness of a case management and housing program in reducing the use of urgent medical services among homeless adults with chronic medical illnesses.
Impact: For every 100 homeless adults offered the intervention, the expected benefits over the next year would be 49 fewer hospitalizations, 270 fewer hospital days, and 116 fewer ED visits.

Goal: The mission of the program is to work vigorously to free participants from the burden of welfare dependency, and achieve a better, happier lifestyle through self-sufficiency. It will serve the taxpayers of Riverside County by reducing welfare dependency, thus making tax dollars available for other expenditures and needs.
Impact: The program produced a large net savings to the government through increased tax revenues and reduced welfare and food stamps payments (as an estimate, $2.84 saved for every $1.00 invested over five years).
